MINE FATALITY – On August 24, 2023, a miner died while preparing a wash plant for relocation. In the process of lowering the feed box into the travel position, the miner was pinned between the feed box and the handrail.

Best Practices
  • Block machinery components against motion before beginning maintenance or repairs.
  • Do not work under suspended loads.
  • When conducting a non-routine task, review safe procedures before starting work and ensure all safety components are in place and working.

This is the 29th fatality reported in 2023, and the 13th classified as “Machinery.”

MINE FATALITY – On October 7, 2022, a mechanic was fatally injured when he was engulfed in material after he entered an air separator to clear a blockage.

Best Practices:
  • Install mechanical flow-enhancing devices, such as mechanical vibratory devices, that can maintain material flow and prevent the need for miners to enter a confined space.
  • Mine operators should establish procedures to clear blockages and train all miners in the procedures.
  • Make sure miners wear a safety belt or harness equipped with a lifeline when entering a confined space, and that a second miner, similarly equipped, is attending the lifeline.

This is the 24th fatality reported in 2022, and the first classified as “Confined Space.”

MINE FATALITY – On March 12, 2021, a miner was fatally injured while attempting to insert a steel pin into a spud beam.

Best Practices:
  • Always assure hoisted equipment movement has stopped and the hoist operator has set the brake before working on hoisted equipment.
  • Assure the hoist operator can see miners working on hoisted equipment.
  • Establish an effective communication protocol, which includes confirmation of instructions, between the hoist operator and miners working on hoisted equipment.
  • Position yourself in a safe location to maintain balance and protection from any energy of cantilevering tools or objects.
  • Stay in a Safe Zone when working around cables and sheave wheel systems.
  • Always maintain a work area that is clean and clear of debris.
  • Train equipment operators in the safe performance of their tasks and potential hazards.

This is the seventh fatality reported in 2021, and the first classified as “Handling Material.”
